I______________________________________________ have been informed and understand that any mishandling, misuse or improper care by myself or anyone appointed by me to care for, handle, or train the above named dog, or training of above named by other than Von Falconer K-9 Training personnel without written permission from Eric Falconer releases Von Falconer K-9 Training, Eric Falconer, or any appointed Von Falconer instructor or employee from any and all liability concerning the above named dog.

I______________________________________________ have been informed and understand that my family (when applicable) and I are required to attend 10 hours of training with the above dog to learn proper handling and control techniques.  I understand that this training must be completed within 60 days of me taking possession of the above dog.

I______________________________________________ have been informed and understand that failure to complete the 10 hours of required training within the first 60 days after taking possession of the above dog will void all Von Falconer K-9 Training contracts, guarantees and agreements, and will also release Von Falconer K-9 Training, Eric Falconer, or any Von Falconer instructor or employee from any and all liability concerning the above dog.
